About Fertility Friends Foundation Grants:

Fertility Friends Foundation offers $ 5,000 grants to individuals and couples across Canada pursuing IVF, egg or sperm donation, or gestational surrogacy.

FFF grants cannot be applied to past expenses, but are available to support upcoming treatments.

WE DO NOT COVER EXPENSES FOR:

Grants are disbursed during the months of June (Spring Applicants) and December (Fall Applicants), so your treatment must begin after those dates.

PLEASE READ –  Important Considerations

• All sections of the application form are presented in English, followed by a French translation.
• When filling out the grant application form please note that: “Applicant” should be the individual who will receive the fertility treatment (if applicable). If you have a partner, their information need to be added in the partner section.
• A non-refundable application fee of $50 is required. You will be redirected to the payment page once your application is submitted.
• Whether you are applying for a Fertility Friends Foundation fertility grant or seeking support for PGT-M testing through the PGT-M Access Fund, you must submit this medical evaluation form to your physician for completion. https://fertilityfriendsfoundation.com/medical-evaluation/
• If your application is selected for further evaluation, you will be contacted by email and asked to provide additional documents (as outlined in the checklist).

Authorization to Release Medical Information

• As part of your application, you have the option to download and complete the Authorization for Release of Medical Information form, available here (English version / French version).
• This form is not mandatory, but it may be helpful if your physician or clinic requests written consent before submitting your medical evaluation to Fertility Friends Foundation.
• If you choose to use it, please complete the form and provide it directly to your physician or clinic. This authorization simply allows them to share the medical information required for your FFF medical evaluation and may help streamline communication or clarify questions they may have. Your clinic may also use this form at their discretion if they require written consent for the release of medical information.
