Follow these steps and tips to submit a complete, strong application to Fertility Friends Foundation (FFF). Please note that our grants are of the amount of $5,000.
Applications open twice a year, in the spring (April 1-30) and in the fall (October 1-30). The application link and full FAQ are available on our website during the open window.
Eligibility (read first)
General criteria
- Applicants and their partner (if applicable) must be Canadian citizens or permanent residents and reside in Canada.
- Applicants must have a medical diagnosis of infertility certified in writing by a medical provider, with the following exceptions:
- Applicants who are members of the 2SLGBTQ+ community
- Single applicants
- Applicants must have seen a Reproductive Endocrinologist within the previous 6 months.
- Applicants must not have a medical condition that would pose a serious risk to themselves or (for females) a future pregnancy (e.g., active acute infections).
- Applicants must have the means to offer continued child support (at least one partner employed).
- Applicants must be patients of a fertility clinic in Canada. Grants are disbursed directly to Canadian clinics.
- Treatment must begin within 6 months after receiving the grant.
- Applicants must be applying for upcoming treatment (FFF does not reimburse expenses that have already occurred).
- Grants are one-time; previous recipients are not eligible.
Clinical criteria
- Applicants must be 18–50 years of age.
- Heterosexual couples must meet the ASRM definition of infertility.
- Same-sex female couples: must have completed 6 IUI cycles unless another infertility diagnosis has been made.
Specific grants you can consider
- Day 1 Grant (includes coaching sessions with Day 1 Fertility)
- Not My Tummy Surrogacy Grant (for surrogacy patients, in partnership with Not My Tummy )
- Dr. Shannon Corbett 2SLGBTQ+ Fertility Grant (for members of the 2SLGBTQ+ community)
- The Lucy of Anishinaabe Grant (for members of the Indigenous community)
Step 1 – Complete the application (by the deadline)
Submit the online form before the stated deadline. Late applications or late supporting documents cannot be accepted. FFF grants typically do not cover the full cost of treatment. Please indicate the dollar amount you can contribute.
Step 2 – Tell your story (Personal Statement) in 900 characters
Help us get to know you: your hobbies, work, family context, and why a grant would make a meaningful difference. Be specific, authentic, and concise.
Step 3 – Upload required documents
- Proof of Canadian citizenship or permanent residency
- Photo of the front of your provincial health card (e.g., OHIP)
Step 4 — Pay the application fee
A $50 non-refundable application fee is processed through our payment platform (visible after you submit your application). Please note that application fees are non-refundable and are not tax-deductible.
Step 5 — Arrange the medical evaluation
A secure physician evaluation link is provided inside the grant application. You are responsible for sending the link to your clinic/physician; they must submit the form directly to FFF.
After you submit
- You will receive an email approximately two weeks after the deadline indicating whether your application has been selected for further review.
- If not selected for further review, you’re welcome to apply again in a future intake.
If you are selected for further review
Please be prepared to provide:
- T4 (Statement of Remuneration Paid)
- Credit score (free options are available from major bureaus)
If you are approved
- FFF will contact you to discuss your grant approval approximately 4 weeks after the application deadline.
- We may request additional documents to verify eligibility (e.g., private insurance details, background check).
- Funds are paid directly to your Canadian clinic and apply to upcoming expenses and treatment only.
- You must start treatment within 6 months of receiving the grant.
- Marketing & Communications: We’ll request a photo, a short written testimonial, and (optionally) a brief video sharing what the grant means to you.
Common mistakes to avoid
- Missing the deadline or leaving required fields blank
- Physician form not submitted
- Missing proof of citizenship/permanent residency or health card image
- Requesting funding for past expenses (FFF cannot reimburse)
Final tips
- Double-check that all documents are uploaded and readable.
- Ensure your contact and clinic details are accurate.
- Watch your email (including spam) after the deadline.
